Yes, Medicare covers CBD oil, with conditions. As of April 1, 2026, Medicare has a program that can cover CBD oil and other oral CBD products for eligible seniors at no charge. Here's the complete answer on how it works, who qualifies, and what to do next.

How the Medicare CBD Program Works
The Medicare CBD program is called the Substance Access BEI (Beneficiary Engagement Incentive). It operates through CMS Innovation Center model organizations, not through standard Medicare drug coverage. Your doctor's organization (called an ACO, or Accountable Care Organization) can sign up to provide qualifying oral CBD products directly to eligible patients. No pharmacy. No reimbursement claim. Your doctor hands you the product.
Three types of organizations can offer the benefit: ACO REACH organizations (serving general Medicare patients), the Enhancing Oncology Model (serving cancer patients), and the LEAD Model (serving long-term care patients, starting January 2027). See innovation.cms.gov for program details.
The annual benefit is $500 per eligible patient. Your doctor's organization absorbs the cost through their program structure. You pay nothing.
Who Qualifies for Medicare CBD Oil Coverage
You need all of the following at the same time: Medicare enrollment, a doctor in a participating ACO REACH or Enhancing Oncology Model organization, that organization must have specifically elected the CBD benefit, and a qualifying condition your doctor determines may benefit from CBD. Not every Medicare patient qualifies yet, participation is growing as more organizations sign up.

What CBD Products Are Covered
Oral products only qualify. CBD oil tinctures are one of the primary qualifying formats alongside gummies and capsules. The product must contain less than 0.3% delta-9 THC and less than 3mg total THC per serving. It must be independently lab tested and from a Farm Bill compliant hemp farm.
What doesn't qualify: CBD flower, pre-rolls, vapes, anything inhaled or smoked. See our complete approved products guide.
How Far Does $500 Go for CBD Oil
At farm-direct pricing, $500 covers considerably more than at premium retail. A 1oz (30ml) CBD tincture at 1500mg CBD retails for $80-120 at specialty stores. At farm-direct pricing, that same supply costs $40-55. At a 25mg daily dose, $500 at farm-direct pricing covers close to 11 months of supply. At premium retail pricing, that same $500 covers roughly 4 months. The pricing source matters enormously for how far your benefit stretches.

What to Do Next
Step 1: Ask your doctor whether their practice is part of an ACO REACH organization or the Enhancing Oncology Model. Step 2: Ask specifically if they've elected the Substance Access BEI benefit. Step 3: If yes, your doctor evaluates whether CBD oil is appropriate for your symptoms. Step 4: If appropriate, they provide qualifying CBD oil to you directly at no charge.
